Aortic Valve Replacement With LIMA to LAD Artery and an Ascending Aortic Replacement [1]

In this video, Dr. Vince Gaudiani demonstrates his technique for aortic valve replacement with a left internal mammary artery (LIMA) to left anterior descending artery (LAD) and an ascending aortic replacement.  

The patient had a 48 mm ascending aorta and severe aortic stenosis with stenosis of the LAD.  

Initially, the LIMA was anastomosed to a large LAD in a side-to-side fashion after a small Fogarty catheter was used to dilate the LIMA.  

The aorta was transected, the valve was excised, and a bioprosthesis was placed, with the method for suture placement demonstrated.   

The sutures were tied down with the Cor-Knot, and a Dacron graft was placed using an inverting continuous suture.
