This is a fabulous video of a miniAVR with a right thoracotomy that is only 3 cm in size and hardly could fit the valve through it! Professor Yan used a series of specialised instruments, a corknot, and an endoscope to achieve this fantastic result. Well done.

The authors retrospectively evaluated the long-term fate of the aortic arch in 168 patients with bicuspid aortic valves who had undergone either ascending aortic (24%) or aortic root (76%) replacement.  At a median follow-up of 5.9 years, the mean aortic arch diameter went from 2.9 cm preoperatively to 3.0 cm at follow-up (p=ns).  The authors conclud

This is a retrospective review of 143 patients requiring reoperation after an initial AVR with a Freestyle® stentless bioprosthesis. Valve survival was 10.4 years for those with structural valve deterioration, but it was much less (4.1 years) if the initial procedure was for infective endocarditis. Operative mortality was <2%.

A medium-size RCT comparing full sternotomy to limited sternotomy for conventional aortic valve replacement, presented at the SCTS Annual Meeting in Glasgow. The 7-10% one year attrition rate seems quite disappointing, and will most probably be discussed. We will await the full paper to see what was the learning curve for the innovative approach.

The authors studied 852 adults with bicuspid aortic valves.  The prevelance of three morphotypes was tallied.  Relative fusion rates were: RL 72.9%, RN 24.1% and LN 3%.  Additionally, 18.3% had no raphe.  Aortic regurgitation was unrelated to morphotype.  Aortic stenosis was more common in the RN fusion group and in the presence of a raphe.  Seventy-
