Coronary disease

A multicenter, randomized, controlled trail including 900 patients divided patients into those undergoing on-pump versus off-pump CABG.  Identical heparinization and heparin reversal protocols were followed.  At angiography at 6 months following CABG, graft patency was inferior after off-pump as compared to on-pump  revascularization.

This video shows an approach to all-arterial grafting that has been developed over the past three decades, and illustrates techniques in four patients who underwent this procedure.

In this paper, the authors aim to further understand the factors associated with vein graft failure (VGF) after CABG. They used data on 1828 participants in the PREVENT IV trial to identify patient and surgical factors associated with VGF.

In this manuscript, the authors use meta-analysis techniques to evaluate the relationship between measured coronary fractional flow reserve (FFR) and patients’ outcomes.

In a cohort of 11,922 diabetic patients undergoing CABG, the authors compare single versus bilateral internal thoracic artery grafting (BITA), complete versus incomplete revascularization, and on versus off-pump CABG.  The authors conclude that BITA with complete revascularization maximizes long-term survival.

This prospective observational study compares patients with coronary artery bypass grafting on aspirin and ticagrelor (n = 173) or on aspirin and clopidogrel (n = 232). Major bleeding complications occurred in 14.5% with ticagrelor and 13.8% with clopidogrel (p=0.89).

This descriptive study presents the peri-operative and 1-year results of on-pump and off-pump surgery performed in patients enrolled in the Arterial Revascularization Trial (ART). Out of the 3102 patients randomised in the ART trial to undergo either single or bilateral mammary artery grafting, 41% had their surgery performed off-pump.

This controlled randomized trial examined the benefits of targeted PCI vs medical therapy alone for patients with stable CAD and stenosis who were found to have zones of fractional flow reserve of less than 0.8 at angiography.  Patients without regions of FFR <0.8 were entered into a  registry.  The composite outcome was death, nonfatal MI, or urg
