The authors demonstrate a quadruple valve replacement in a patient with carcinoid heart disease causing severe regurgitation of all four heart valves and poor left ventricular (LV) function. To date, only 11 quadruple valve replacements have been performed in the United Kingdom, and 85 percent of UK units have no experience with this operation. This case is one of four successful cases undertaken in the surgeons’ unit in the last five years. It is the world’s first use of the Edwards Mitris Resilia valve in the tricuspid position and the world’s first use of Edwards Resilia valves in all four positions.
The authors demonstrate the techniques used, including patch augmentation of the right ventricular outflow tract (RVOT) to facilitate pulmonary valve implantation. The operation was successful, and the patient is now home in NYHA class I.
