This video demonstrates the case of a 48-year-old man who had a Mitral valve repair in 2014 and then presented again six years later with mitral regurgitation. After discussion, the patient chose to have a mitral valve replacement. The procedure began with right femoral vein cannulation and direct aortic cannulation, and the IVC and SVC were occluded. The right atrium was opened, as this was the easiest approach.
Sondergaard’s groove is considered the most difficult area to access in redo surgery in Dr. Gaudiani’s opinion. The fossa ovalis was opened, and a good view of the mitral valve was obtained. The ring was first removed, and the P3 leaflet was found to be bulging, as this leaflet did not have any supporting cords.
The Onyx mechanical mitral valve was used for this operation, with sutures placed from the atrium to the ventricle. The cords were also spared, and this technique is shown in the video.
