In this video, Dr. Vince Gaudiani demonstrates his method for repairing bileaflet mitral valve prolapse through a rooftop incision in the left atrium using an upper ministernotomy.  

The repair is performed on a Barlow’s valve with prolapse of both the anterior and posterior mitral leaflets. There were no torn chords to the anterior leaflet, so chords were placed using the Chord-X premade chordal placement system, which offers three pairs of premade chords. The chords were placed 4-5 mm up on the posterior leaflet, which is an alternative to resecting leaflet tissue.  A SimuPlus ring was placed, and the postoperative result is excellent.
