Valve-Sparing Aortic Root Replacement in Bicuspid Valve With Papillary Fibroelastoma

This video demonstrates a valve-sparing root replacement (David procedure) with removal of the papillary fibroelastoma and reconstruction of the valve using 180° commissure orientation and cusp plications. A 34 mm straight Dacron graft was used. The base of the prosthesis and the coronary ostia were marked with radiopaque markers.

Congratulations for this very nice video and procedure. What is your position with long term results in this particular case. Bicuspid valves with plication may have reduced long term results especially in patients with annulus over 30 mm. Regards
