This video features a 42-year-old male patient who received an On-X mechanical valve.  

An aortotomy was performed after a hemisternotomy, and then the leaflets were removed as closely to the annulus as possible.  

Dr. Gaudiani notes that the On-X valve is large; therefore, the sizer must fit easily into the annulus. Additionally, he mentions that this mechanical valve can be placed as a 21 mm valve but has the flow characteristics of a 25 mm biological valve.  

The opening line was placed perpendicular to the septum, and pledgets were not used. It is also crucial to visualize the left and right ostia after implantation.
