Severe leaflet tethering which is mainly the result of long standing Tricuspid valve regurgitation and RV dilatation is the main predictor of residual and/or recurrent valve insufficiency. We present the Novel technique of Papillary Muscle Cross Control which effectively overcomes the leaflet tethering in a very practical, simple , and reproducible manner by controlled approximation of the anterior papillary muscle to the posterior/septal part of the semi-rigid annuloplasty ring. The papillary cross-control technique is applicable both as a supplementary technique after ring annuloplasty for functional TR with tethered cusps and also for completing the repair in more complex cases of organic valve disease such as Ebstein Anomaly. Our early and late results with this technique has been quite encouraging.
