The optimal timing of tracheotomy in critically ill patients remains a topic of debate. The Authors performed a systematic review to clarify the potential benefits of early versus late tracheotomy. Three categories of timing of tracheotomy (within 4 versus after 10 days, within 4 versus after 5 days, within 10 versus after 10 days) were analyzed.

This video presents a single-port VATS right upper lobe sleeve lobectomy for a stage I carcinoid tumor in a 52-year-old patient. Sleeve lobectomy is a challenging and complex procedure. Typical carcinoids occluding the right upper lobe bronchus present an ideal opportunity for sleeve upper lobectomy due to their indolent nature and excellent oncologic outcomes.

250 children with tuberculosis causing central airway narrowing were evaluated. 34% needed transthoracic nodal dissection for relief of obstruction, nearely one-third of those as an urgent procedure. Success was more common in those with bronchus intermedius or left main bronchial obstruction. The success rate was 98%.

This article describes bronchial thermoplasty, a novel treatment option in uncontrolled severe asthma. The procedure targets reducing the hypertrophied smooth muscle in the airways, which is considered to be part of the pathophysiology of and contributor to asthma symptoms.

LVR using endobronchial therapies remains experimental. This multicenter study tested LVR coils in 60 pts for safety and efficacy. There were a total of 18 serious adverse events. At 6 and 12 mos, dyspnea scores decreased 11%, 6 min walk increased 30-50 m, and FEV1 increased 0.11 L. Pts with either heterogeneous or homogeneous emphysema appeared
