The present a case of a 55 year old male with known mitral valve insufficiency. By transesophageal echo the patient was found to have a ruptured chordae on the P2 segment of the posterior leaflet of the mitral valve. He underwent minimally invasive mitral valve repair via right thoracotomy.

This video shows newer techniques of aortic valve repair: commissural annuloplasty; central plication for prolapse; leaflet pericardial augmentation; and ultrasonic calcium debridement. Five cases are shown, over the spectrum of valve pathologies. Greater use of aortic valve repair seems indicated.

UPDATE: A follow-up video has been added that demonstrates how to manufacture the chordae system. Through a port-access incision, this video demonstrates a technique for placing an echocardiographically pre-measured chordae system to properly suspend the whole of the prolapsed anterior leaflet to its normal level of coaptation.

Artificial chordal replacement allows repair of most bileaflet prolapse and Barlow's valves. This video, presented at the 2004 STS meeting, is still useful today as chords are being used more frequently.

This video reviews techniques of artificial chordal replacement (ACR) as a component of complex mitral repair. A brief summary of ACR history is given, as well as basic techniques of ACR placement and length adjustment. Several cases then are presented, and operative videos are correlated with pre- and post-operative transesophageal echocardiograms.

This video shows the main surgical aspects of the Bentall-De Bono procedure with a new biological composite conduit. This is the case of a 76-year-old man with ascending aorta aneurysm and aortic valve cusp calcification. The composite conduit utilized for aortic root and aortic valve replacement is a prefabricated biological valve, called BioValsalva.

Percutaneous mitral repair using the MitraClip® system is a method derived from the edge-to-edge surgical procedure. The MitraClip® system replaces suturing with a clip to join the free edges of the opposing leaflets at the site of regurgitation.
