TAVR (TAVI)

September 1, 2014
In this retrospective study, the authors evaluate the effect of preprocedural significant mitral regurgitation (MR) on outcomes of patients undergoing transcatheter aortic valve implantation (TAVI) for sever aortic stenosis. In an overall series of 316 patients, 60 had been diagnosed with significant MR prior to the procedure.

The authors of this review article provide an extensive overview of the curent state of play in the field of Aortic Stenosis. They discuss risk scores, treatment decisions, and up-to-date evidence of Transcatheter Aortic Valve Replacement.

The European system for cardiac operative risk evaluation II (EuroSCORE II) and the Society of Thoracic Surgeons Predicted Risk of Mortality (STS-PROM) were validated for overall in-hospital mortality in an US cardiac surgery database comprising >50.000 patients. More reliable results were achieved with the STS-PROM.

In this manuscript, the authors describe their experience with the treatment of 8 high risk patients with pure, severe aortic valve regurgitation, using the self-expandable Acurate TA prosthesis  through a transapical approach.

In a two-part review, Bax and many other authorities on transcatheter aortic valve implantation (TAVI) provide an extensive overview of the open issues related to patient selection, treatment strategy, and procedural aspects that will help further improve outcomes after TAVI.

Bicuspid aortic valve (BAV) stenosis has been regarded as a relative contraindication for transcatheter aortic valve implantation (TAVI).

The consortium of authors of this paper determined the survival after Transcatheter valve in valve implantation inside a failed surgical bioprosthesis. The authors report a 1 month mortality rate of 7.6% and a major stroke rate of 1.7%. One-year survival was 83.2%, and 313 (92.6%) of survivors had a good functional status (NYHA I/II).

The PARTNER trial reported reduced mortality in pts with prior CABG who underwent surgical AVR compared to TAVR.  This study further explores the 288 pts in these two groups.  The groups were similar in their clinical presentation and had similar instances of procedure-related mortality, stroke, and MI.  The TAVR pts had more paravalvular leak, at 2

The incidence of coronary artery disease among patients with severe aortic stenosis and candidates for TAVI ranges from 40 to 75%. In the absence of randomized trials, the management of these patients remains to be established. In this paper, the authors offer a good overview of the subject and propose an algorithm for treatment.

In this study the authors use data from 1432 patients included in the German TAVI registry to identify variables which predict poor outcome in patients left with more than mild aortic regurgitation (AR) following TAVI.  The Medtronic Core Valve prosthesis had been used in the great majority of the procedures.
