The resection of locally advanced lung cancer is generally accomplished with thoracotomy. When possible, the authors use a hybrid approach. This video illustrates two cases of invasive right upper lobe tumors. VATS exploration through a standard anterior, two-incision, approach directs a secondary counter incision.

Since video-assisted thoracoscopic surgery (VATS) was described for anatomic lobectomies, many technical variations have been published. The uniportal technique, which enables the entire procedure to be performed through a single port, was recently introduced. This video shows a uniportal VATS lobectomy for lung cancer treatment.

Radiotracer-guided localization of pulmonary nodules was introduced in 2000, with subsequent studies confirming its reproducibility and reliability. As more frequent small, asymptomatic masses are being identified, this technique could prevent unnecessary thoracotomies and preserve lung parenchyma.

The anterior cervicothoracic approach was used to remove a desmoid tumor of the thoracic outlet in a 70-year-old male. The chest wall was reconstructed with titanium bars and a polytetrafluoroethylene-patch. The clavicle was fixed using a titanium clip. The postoperative period was uneventful.
