Dual incision thoracoscopic (DIT) decortication for stage 2 fibrinopurulent effusion is a further reduction in the invasiveness of thoracoscopy. DIT decortication is less invasive in a patient who is already morbid with infection, and the limited incisions or ports further reduce nerve injury in a relatively crowded rib space.

The resection of locally advanced lung cancer is generally accomplished with thoracotomy. When possible, the authors use a hybrid approach. The cause of pectus deformities is thought to be abnormal elongation and/or distorsion of the rib cartilages. A simplified open repair is described based on the subperichondrial resection of the abnormal cartilages, and in the case of pectus excavatum, the use of an easily removable metallic strut.

The anterior cervicothoracic approach was used to remove a desmoid tumor of the thoracic outlet in a 70-year-old male. The chest wall was reconstructed with titanium bars and a polytetrafluoroethylene-patch. The clavicle was fixed using a titanium clip. The postoperative period was uneventful.

A young female with a single metastasis at the level of the upper portion of the sternum underwent surgical excision of the sternum followed by sternal allograft implantation. The use of a sternal allograft provides excellent functional and cosmetic results.
