The authors prospectively performed coronary angiograms in their patients undergoing isolated CABG who demonstrated a rising cardiac troponin I profile in the first 12 hours following heart surgery - despite being asymptomatic.  Among a total of 1693 patients from 2005-2011, 29 (1.7%) of patients had a rising CTnI.

Repair of this congenital defect in 34 pts was associated with no early mortality and substantial improvement in LV function (EF, shortening fraction, LVEDD).  No postoperative circulatory support was required.

The authors of this article compared CABG and PCI with second generation drug eluting stents (DES-PCI) in patients with multivessel coronary artery disease. They used data from a large observational registry and applied propensity score matching to create groups with similar baseline characteristics.

Benedetto and co-authors analyzed propensity-matched groups of patients who underwent CABG or PCI (n= 1097 pairs) for multivessel disease, and found that outcomes were comparable at 1-year follow-up, while CABG did significantly better during follow-up to 5 years and Kaplan-Meier curves continued to diverge even further after 5 years.

This is a very provocative British study that examined whether a more liberal (Hgb < 9 g/dL) versus a more restrictive (Hgb < 7.5 g/dL) transfusion threshold after cardiac surgery is associated with higher morbidity and costs.

This interesting article mapped myocardial remodeling in swine after induced MI using serial 3D MRI imaging.  Principal strain magnitude and angle were measured, and demonstrated progressive decrease in magnitude and angle rotation away from the site of injury, resulting in adverse remodeling and decreased contractility.

Comparing 714 on pump and 714 off pump CABG patients, the investigators investigated the incidence of Acute Kidney Injury (AKI) and whether cardiopulmonary bypass time and or duration were independent contributors to AKI.  The investigators concluded that there are no differences in the incidence of AKI during  CABG surgery employing CPB.

This study describes the development and validation of a risk score for post-operative mortality in patients undergoing surgical ventricular reconstruction (SVR). The study included data on 525 patients who underwent SVR at a single institution since 2001. Most patients (93%) had cocomitant CABG and 27% of patients had concomitant mitral surgery.
