Over 1400 patients with asymptomatic severe carotid stenosis were randomized to with carotid stenting or endarterectomy.  The endpoints were the composite of death, stroke, or acute MI within 30 days, or ipsilateral stroke.   Stenting was non-inferior to endarterectomy.   The rate of the composite endpoint was 3.8% vs 3.4% (stent vs surgery).  At 30

This analysis compared results of open repair for chronic Type B dissection (OAR) and endovascular repair (TEVAR) using propensity matching techniques.  Freedom from rupture/reintervention at 10 years was 78%.  Specific outcomes were similar for both types of intervention, but overall treatment efficacy was better for OAR.

Open repair is the standard for aneurysmal degeneration of the arch. Endovascular techniques offer an alternative for patients with prohibitive comorbidities. This video describes a complete endovascular arch reconstruction using a modified stent graft approach, combined with cervical debranching.

The Columbia group performed a retrospective analysis of 46 balloon atrial septostomies in 32 patients with severe and medically refractory PAH from 2002-2013.   There were no procedural deaths or complications. BAS was safely used as a bridge to lung transplantation or to alleviate right heart failure symptoms and/or syncope.

This retrospective review summarizes a single institution experience with thoracic aortic endovascular stenting in 420 pts over 20 years.  Indications were aneurysm (238), dissection (100) and trauma (39).  78% of pts were deemed to be at high risk for open repair.  Mortality was 4.8%, stroke incidence was 5%, and cord ischemia was temporary in 7.9%

This double-blind placebo-controlled trial involved 153 pts undergoing elective endovascular aortic aneurysm repair, of whom half received methylprednisolone preoperatively.  The intervention reduced the systemic inflammatory response from 92% to 27%, plasma IL-6 was decreased by 90%, and CRP, body temperature, IL-8, and TNF were also reduced.  MMP-9

Stepped wedged cluster randomized control trial from the World Health Organization comparing outcomes in those who received a Checklist.  Implementation of the WHO Surgical Safey Checklist decreased morbidty and hospital length of stay.

This report summarizes outcomes in 40 reoperated patients (mean age 33 years) of an original 122 who underwent initial aortic root surgery for Marfans. Reoperations took place a mean of nearly 10 years later. 5 and 10 year survivals after second operations were 80% and 66%. Open and stent grafts procedures did not appear to have different outcomes.
