The cause of pectus deformities is thought to be abnormal elongation and/or distorsion of the rib cartilages. A simplified open repair is described based on the subperichondrial resection of the abnormal cartilages, and in the case of pectus excavatum, the use of an easily removable metallic strut.

Totalis Situs inversus (SIT) is an extremely rare condition. There are few reports in literature describing thoracic surgery in patients with this unusual anomaly. This video is the first report of a thoracoscopic lung resection of a pulmonary mass in a patient with SIT.

Professor Robert J. McKenna, Jr., MD is Medical Director of Thoracic Surgery and Trauma at Cedars-Sinai, Los Angeles, CA. He pioneered the anterior approach for VATS lobectomy and has by far the largest experience in this technique. However, he has also served as the principal investigator on several trials, including the Z0030 trial on lymph node staging and also the NETT Trial.

The anterior cervicothoracic approach was used to remove a desmoid tumor of the thoracic outlet in a 70-year-old male. The chest wall was reconstructed with titanium bars and a polytetrafluoroethylene-patch. The clavicle was fixed using a titanium clip. The postoperative period was uneventful.

This video demonstrates the ligation of the thoracic duct by a VATS approach. The patient suffered damage to the duct in the neck resulting in neck swelling and made local repair of the duct impossible. In this video the author demonstrates his technique for ligating the duct via a posterior VATS approach.
