Twenty years ago, Dr. William Walker performed a left upper lobectomy using a 3-port approach standing in the posterior position. Now approaching his thousandth operation, he discusses his first VATS lobectomy and his current techniques.

Demonstrated in the video is a novel concept for facilitating extensive neochordal repair in mitral regurgitation based on the paradigm “respect rather than resect”. Both leaflet tissue and motion are respected. The leaflet is folded and the neochordae is tied, maintaining free margin and annulus edge-to-edge.

The RV-PA conduit or Sano modification, has recently been reintroduced into the Norwood armamentarium. Many centers, however, have been experiencing conduit obstruction with the Sano. This video demonstrates a modification of the technique used for the RV-PA conduit that has reduced the incidence of conduit obstruction at our institution.
