In this video, Dr. Vince Gaudiani demonstrates how to perform an Onyx Chord-X chordal repair for a P2/P3 prolapse. He began by cannulating the femoral vein, then performed an upper mini sternotomy before placing a right superior pulmonary vein vent and aortic cannulation with antegrade cardioplegia. The roof of the left atrium was then opened, and stay sutures were placed to optimize the view. All annuloplasty sutures were placed before assessing the valve. 

During the assessment of the valve, stretched chords were found at P2 and P3. Therefore, the 12mm the Onyx Chord-X system was used to place three sets of chords from the posteromedial papillary muscle to P2 and P3. After this, a 35mm posterior annuloplasty ring was placed, and the left atrial appendage was resected with a stapler.

Very impressive technique: I have done over 1000 mini upper sternotomies for AVR and am now contemplating using it for MV repairs. I used to amputate the LA appendage and oversew it fearing that a stapling line would leak. Could you indicate which stapling device you use?
Thx for your note. We use the endo GIA stapler with 60 mm head and 4.8 or medium staples. CTS will be publishing another 10 or 12 mitral repair videos over the next few weeks. Our Editor, Joel Dunning, noted that if you put sterile gel on the stapler head, it prevents unused staples from falling in the field.
