Chordal Repair of P2 and P3 and Left Atrial Appendage Stapling Through an Upper Mini Sternotomy
Gaudiani V, Tsau P. Chordal Repair of P2 and P3 and Left Atrial Appendage Stapling Through an Upper Mini Sternotomy. September 2024. doi:10.25373/ctsnet.26972596
In this video, Dr. Vince Gaudiani demonstrates how to perform an Onyx Chord-X chordal repair for a P2/P3 prolapse. He began by cannulating the femoral vein, then performed an upper mini sternotomy before placing a right superior pulmonary vein vent and aortic cannulation with antegrade cardioplegia. The roof of the left atrium was then opened, and stay sutures were placed to optimize the view. All annuloplasty sutures were placed before assessing the valve.
During the assessment of the valve, stretched chords were found at P2 and P3. Therefore, the 12mm the Onyx Chord-X system was used to place three sets of chords from the posteromedial papillary muscle to P2 and P3. After this, a 35mm posterior annuloplasty ring was placed, and the left atrial appendage was resected with a stapler.
