In this video, Dr. Vince Gaudiani demonstrates how to perform an Onyx Chord-X chordal repair of a P1 prolapse. He began by cannulating the femoral vein and then performed an upper mini sternotomy. Following this, he placed a right superior pulmonary vein vent and performed aortic cannulation with antegrade cardioplegia.
The roof of the left atrium was then opened, and stay sutures were placed to optimize the view. Before assessing the valve, all the annuloplasty sutures were placed.
Upon assessment of the valve, stretched chords were found at P1 with a billowing leaflet. Therefore, the 16mm Onyx Chord-X premade system was used to place three sets of chords from the posteromedial papillary muscle to P1. After this, a Duran posterior annuloplasty ring was placed, and the atrium was closed.
