This patient had an emergency mitral valve repair at another institution, but the repair failed early. This resulted in P1 and P2 prolapse and systolic anterior motion of the leaflets, with the anterior leaflet abutting the hypertrophied septum.  

After cannulation, cardioplegia, and cross-clamping, the aorta was opened, and a relatively conservative amount of the left ventricular septum was resected.  

The roof of the left atrium was then opened to access the mitral valve, and the Onyx Chord-X system was used to place 4 x 12mm chords to the posterior and anterior leaflets. Then, a flexible annuloplasty ring was placed to complete the operation.
