Cerebral Protection

March 19, 2018
Common carotid artery cannulation can provide antegrade blood flow to the whole body during cardiopulmonary bypass, as well as maintain cerebral perfusion even at the time of arch exclusion or repair.

In this video, the authors present the case of a patient with an extensive thoracic aorta aneurysm, who was treated with a two-stage hybrid approach.

This single-center study explored the relationship between spontaneous echo contrast (SEC) and the incidence of thromboembolism and CVA in 98 patients undergoing peripheral VA ECMO.  Twenty-two percent of patients had SEC while they were on ECMO.  Those exhibiting SEC had significantly higher rates of intracardiac thrombus (46% v.

Not only an authoritative 16 year retrospective report on 957 planned aortic aneurysm surgeries from Cleveland Clinic, but a stimulating critique ( pages 771-774) with Tyrone David who was the invited discussant of the manuscript at its presentation at the AATS last April.

Deep hypothermia and cardioplegic myocardial arrest, with or without selective cerebral perfusion, is traditionally performed during aortic arch reconstruction. In this video, the authors describe a technique for selective cerebral perfusion combined with controlled coronary perfusion for hypoplastic aortic arch repair.

This video illustrates the technique used to repair an aortic dissection with a tear involving the aortic root contiguous with the right coronary artery (RCA). In addition to the dissection repair, an aortic root replacement and a jump graft to the RCA are performed.

A technique for the treatment of a complex aortic disease is demonstrated in this video.  The aortic arch was replaced followed by deployment of a thoracic stent-graft through the branch of a Dacron graft.

Damiano is a pioneer in the area of minimally invasive cardiac surgery. His developmental work on robotically assisted microsurgery for coronary artery bypass grafting (CABG) earned him a ComputerWorld Smithsonian Award in June 1997. Damiano performed the first robotically assisted CABG procedure in North America in December 1998.
