Transplantations of hearts from donors >50 years (n=26) were compared to 136 transplantations of hearts from younger donors. Hearts from older donors were preferably transplanted to older recipients, who had also longer waiting times.

Systolic and diastolic myocardial motion was determined using magnetic resonance tissue phase mapping (n=27). Cold ischemic time of the transplanted heart >155 min is associated with decreased myocardial velocities whereas longer postoperative time has a positive influence.

The authors report outcomes for recipients of heart transplants performed more than 20 years prior. Overall 20 year survival was 56%. Causes of mortality were similarly distributed among rejection, malignancy, infection, and allograft vasculopathy. Only 2 patients underwent retransplantation.
