3D echo was used to preoperatively assess the likelihood of recurrent MR after ring annuloplasty for ischemic MR.  Recurrence was asssociated with P3 tethering angle greater than 29.9 degrees (accuracy 92%).  The authors suggest that valve replacement rather than ring annuloplasty should be considered in such patients.

In a propensity-matched analysis of the national database of Sweden, Glaser et al found that survival among patients aged 50-69 who underwent aortic valve replacement was significantly better in those receiving a mechanical versus biological valves. In a subgroup analysis this was evident in patients aged 50-59 but not in those aged 60-69.

This video shows a right axillary Edwards SAPIEN 3 TAVR implanted through the new apical Certitude delivery system, in a patient with severe aortic stenosis. The postoperative follow-up was uneventful and the patient was discharged home on postoperative day three.

2 year outcomes of a randomized trial of MV repair vs MV replacement for severe ischemic MR are reported.  LVESVI was similar between the two groups.  Recurrent moderate or severe MR was higher in the repair group (59% vs 4%).   The repair group suffered higher rates of heart failure and hospital readmission.  Mortality rates were not different.
