Coronary Disease - Surgery

April 7, 2015
The authors prospectively performed coronary angiograms in their patients undergoing isolated CABG who demonstrated a rising cardiac troponin I profile in the first 12 hours following heart surgery - despite being asymptomatic.  Among a total of 1693 patients from 2005-2011, 29 (1.7%) of patients had a rising CTnI.

Repair of this congenital defect in 34 pts was associated with no early mortality and substantial improvement in LV function (EF, shortening fraction, LVEDD).  No postoperative circulatory support was required.

Benedetto and co-authors analyzed propensity-matched groups of patients who underwent CABG or PCI (n= 1097 pairs) for multivessel disease, and found that outcomes were comparable at 1-year follow-up, while CABG did significantly better during follow-up to 5 years and Kaplan-Meier curves continued to diverge even further after 5 years.

In this manuscript the authors present a post-hoc study of the SYNTAX trial. They compare outcomes between those patients on optimal medical therapy (OMT) and non-optimal medical therapy following revascularization for complex coronary artery disease (CAD).

Comparing 714 on pump and 714 off pump CABG patients, the investigators investigated the incidence of Acute Kidney Injury (AKI) and whether cardiopulmonary bypass time and or duration were independent contributors to AKI.  The investigators concluded that there are no differences in the incidence of AKI during  CABG surgery employing CPB.

The authors evaluated the change in risk profile of 65097 patients who underwent CABG between 1997 and 2011 in the Veterans Affairs Surgical Quality Improvement Program (VASQIP).

Dr. Reed et al from the Cleveland Clinic conducted this prospective study to assess whether VerifyNow point-of-care platelet testing could predict bleeding in 39 patients receiving Plavix and undergoing CABG.  Patients in higher PRU quartiles experienced smaller decreases in hematocrit and less bleeding.

In this recently published clinical study investigators from Columbia University, New York Presbyterian Hospital evaluated the safety of Del Nido cardioplegia in high-risk CABG surgery after AMI.

Propensity-score matched patients undergoing PCI or CABG in regional centers in Emilia-Romagna, Italy, 2002-2008 were evaluated for long-term outcomes.

This systematic review and meta-analysis explores the literature to determine the relative advantages and disadvantages of using bilateral IMAs over single IMAs on outcomes after CABG in diabetic patients.
