This report presents trends and outcomes of cardiac surgery following initial transcatheter aortic valve replacement (TAVR). The Society of Thoracic Surgeons (STS) Adult Cardiac Surgery Database was queried for all adult patients undergoing cardiac surgery after a previously placed TAVR between January 2012 and March 2023.

Watch EIC Joel Dunning talk about the advantages of SAVR over TAVR and the data supporting the procedure in the right patients with Prof. Leonard N. Girardi.
February 23, 2024
In this series of videos, Dr. Vincent Gaudiani shares his experience in the new field of surgery after previous TAVR and provides his tips for safely performing redo surgery for a variety of indications including TAVR early and late failure, endocarditis, and when further valve surgery is required.

A recent study by Jad Malas et al. comparing transcatheter aortic valve implantation (TAVI) and surgical aortic valve replacement (SAVR) in patients less than 60 years of age was presented during the 2024 STS Annual Meeting in San Antonio, Texas.
January 25, 2024
The first video shows a contained rupture of the aortic annulus, which was addressed with removal of the TAVR and a Dacron patch over the rupture. The second video demonstrates a TAVR explantation in a patient who had previously undergone CABG and TAVR, culminating in a mitral valve replacement.

Stroke after transcatheter aortic valve implantation (TAVI) is associated with considerably worse postinterventional outcomes. However, predictors of stroke and the long-term risk after TAVI remain unknown.
January 11, 2024
Preinterventional frailty assessment is gaining relevance in both cardiac surgery and transcatheter aortic valve implantation (TAVI). Nowadays, frailty assessment is an essential part of the heart team discussions when evaluating patients for cardiac surgery and transcatheter interventions.
January 11, 2024
The first video shows the removal of a Sapiens TAVR, which had become stenotic, and a replacement of a dilated ascending aorta through a mini sternotomy. The second video shows redo valve surgery in a patient who had a surgical AVR then a TAVR in place and required SAVR with left ventricular outflow tract reconstruction after removal of both the TAVR and the previous SAVR.

Degenerative aortic valve stenosis is the most frequent valvular heart disease in western countries, and valve replacement is the gold standard for symptomatic severe cases.
November 10, 2023
In this recording from the 2023 Transcatheter Heart Valves Greece conference, Dr. Michael J. Reardon, a cardiothoracic surgeon at Methodist DeBaket Heart and Vascular Center, discusses varying confidence in the shift in TAVI guidelines from operative risk to age.
