TAVR (TAVI)
November 16, 2017
This brief article compares costs and morbidities for TAVR and SAVR in the current era. TAVR costs are higher than for SAVR and have increased despite no apparent change in comorbidity index. In contrast, SAVR costs remained stable during the same period.

Severe aortic regurgitation without calcification of the aortic annulus cannot be treated with transcatheter implantation of a balloon-expandable valve, however, self-expandable valves can be successfully anchored in the absence of annulus calcification.

Many answers need to be addressed before transcatheter aortic valve implantation (TAVI) is adopted in lower risk patients. This paper highlights the pros and cons of TAVI based mostly on randomized clinical trials involving the two device platforms approved in the United States.

This study evaluates 1028 octogenarians who underwent isolated aortic valve replacement (AVR, 306 TAVR and 722 SAVR) between 2002 and 2015 at Brigham and Women’s Hospital in the US. The mean age of patients was 84.4 ± 3.4 years old, with overall STS-PROM score of 5.96 ± 3.87.

The authors analyzed their prospective hospital TAVR register to assess whether the addition of serum albumin as a marker of frailty would improve the ability of the STS and EuroSCORE-2 scores to predict mortality. TAVR patients were divided into 4 groups based on median serum albumin and median STS and EuroSCORE-2 values.

Vahidkhah and colleagues used patient-specific geometry of the aortic root, ascending aorta, and coronary arteries to model blood flow near the aortic valve leaflets. The authors use this model to better understand flow-related permissive factors that might mediate leaflet thrombosis following valve replacement.
