VAD

The authors of this review article offer an approach to the utilization of a contempory mechanical circulatory support devices.  They further provide insights of the key role that well-functioning interdisciplinary teams contribute to the success of a MCS program and future directions to consider as this field continues to rapidly evolve.

Rajamiyer Venkateswaran of Mythenshawe Hospital, Manchester, UK, discusses temporary mechanical circulatory support as a bridge to transplantation in cases of cardiogenic shock.

With the everpresent question of potentially extending LVAD therapy to ambulatory patients with advanced heart failure, the authors--using a Markov model--projected the incremental cost-efffectiveness ratio (ICER) of DT LVADs in this population as compared to conventional medical therapy.

It’s well known that VAD and ECMO are associated with bleeding and decreased platelet activity.  This study investigated whether loss of platelet receptors occurred in 21 VAD patients and 20 ECMO patients.

With the rapid expansion of mechanical circulatory support (MCS) in various contexts (CVICU, in-hospital cardiac arrest (E-CPR) and the ED), the authors have provided a very nice overview of the general principles of MCS options and management to assist the broader healthcare team involved with these complex patients.

ECMO is widely used for cardiogenic shock treatment.  Since peripheral V-A ECMO will increase LV afterload and IABP therapy can counterbalance it, will IABP combined with ECMO improve the outcomes?

Norepinephrine is often used to maintain the mean pressure during open heart surgery but it is said that it could constrict cerebral arteries, reducing cerebral blood flow. Is it true? The authors explored the association of Norepi doses and rSO2 measured using near-infrared spectroscopy.

As the Authors have stated: "maximal medical therapy can no longer be seen as a justifiable end‐point for refractory circulatory shock, at least in well‐resourced health settings." To frame their discussion on mechnical circulatory support (MCS) in the current era, they have provided a case vignette for options potentially available to the patient wi

This is a very interesting and provocative article that discusses Exracorporeal Cardiac Life Support (ECLS) as an adjunct to CPR. ECLS has gained in popularity with new tools and techniques, ECLS guidelines, protocols and better understanding of the importance of early initiation.
