Aortic valve disease

February 13, 2017
The authors report on an echocardiographic sub-study of the NOTION trial, a prospective randomized study which compared outcomes between 120 patients undergoing TAVI and 112 patients undergoing aortic valve replacement (AVR).
January 31, 2017
The authors present a multidisciplinary minimally invasive approach to aortic valve replacement.
December 27, 2016
The authors report on their findings an ongoing Japanese multicentre prospective registry. They evaluate outcomes in 478 patients undergoing transfemoral  TAVI and treated with percutaneous puncture using an expandable sheath, Edwards Sapien XTprosthesis and a Perclose ProGlide system.

This retrospective study compared Celsior (65% of pts) to all-blood continuous myocardial protection (35%) in 631 patients undergoing elective aortic valve replacement.  Troponin T release was lower in Celsior patients, as was the risk of postoperative death (OR 0.33; 0.15 - 0.76).  Other outcomes were similar between the groups.

This video demonstrates a valve-sparing root replacement (David procedure) with removal of the papillary fibroelastoma and reconstruction of the valve using 180° commissure orientation and cusp plications.
December 3, 2016
In this manuscript, the authors offer a comprehensive and thorough review of the pathophysiology of aortic stenosis and relate this to the different potential medical treatment options to slow down the progression of the disease.

Minimally invasive aortic valve replacement represents an increasingly acceptable alternative to standard sternotomy with potential reductions in morbidity.  There are technical nuances of the minimally invasive approach in which sutureless valves may offer an advantage.
November 28, 2016
An extremely important journal article comprised of two case reports involving valve perforation and insufficiency following use of the Cor-Knot Automated Fastener (LSI Solutions, Victor, NY, USA) during valve replacement.
November 23, 2016
Early single institution experience with minimally invasive sutureless aortic valve replacement was reported.  Among 300 patients, surgeon-specific and institution-specific learning curves were evaluated for technical success and 30-day complications.  A cluster of complications occurred early in the experience and then standardized.  No significant

Valve in valve transcatheter aortic valve implantation (ViV-TAVI) is an alternative to redo surgical aortic valve replacement (SAVR) in inoperable patients and inpatients at high operative risk.
