Minimally Invasive

April 7, 2015
The authors prospectively performed coronary angiograms in their patients undergoing isolated CABG who demonstrated a rising cardiac troponin I profile in the first 12 hours following heart surgery - despite being asymptomatic.  Among a total of 1693 patients from 2005-2011, 29 (1.7%) of patients had a rising CTnI.

The authors of this article compared CABG and PCI with second generation drug eluting stents (DES-PCI) in patients with multivessel coronary artery disease. They used data from a large observational registry and applied propensity score matching to create groups with similar baseline characteristics.

In this discussion filmed at the EACTS 2014 Annual Meeting in Milan, Italy, Marko Turina and Mattia Glauber consider minimally invasive valve replacement, with a focus on the Perceval S sutureless valve.

This propensity matched single institution study compared conventional aortic valve replacement via a full sternotomy (CAVR) to minimal access aortic valve replacement (MAAVR) via an upper hemisternotomy. Over the eleven year study period a total of 2103 patients underwent isolated aortic valve replacement.

This video shows a very rare vascular injury occurring during VATS left upper lobectomy, and its repair. The case suggests some of the limitations of VATS procedures.

This study propensity-score matched patients from a cohort who underwent resection for stage I or II lung cancer and compared 175 pairs who underwent VATS or open lobectomy.  The VATS group experienced shorter hospital length of stay, fewer complications, and more frequent discharge to home.  Survival was similar.

This is an interesting review of the current status of hybrid coronary revascularization (HCR). The authors discuss the merits and disadvantages of simultaneous versus staged procedures and describe the individual components of HCR. They also analyze the current evidence with regards to results and suggest indications for this type of therapy.

This article shows that MIMVS is a safe approach with low morbidity and mortality that allows a high and durable repair rate with low reoperation rates and favorable short-term event-free survival

This study randomized 100 patients with AS to full sternotomy AVR with a conventional prothesis or hemisternotomy with a rapid deployment prosthesis.  CPB times were similar, but Ao crossclamp times were 24% shorter in the rapid deployment group.  Acute outcomes were similar.  The rapid deployment group had a lower transvalvular gradient and a lower
