Mitral valve disease

An interesting case that raises the question of the need for anticoagulation in patients treated with a transcatheter valve in valve in mitral position

Appropriate management of functional ischemic MR at the time of CABG is unknown.  This study randomized 31 patients with moderate functional ischemic MR to CABG alone or in combination with ring annuloplasty.  Moderate differences in outcomes were evident at 3 months, but at 12 months the groups were similar in LVEF and MR improvement.

Pts undergoing aortic root replacement who moderate MR preoperatively were evaluated for outcomes after mitral repair vs no repair. Mitral repair was associated with longer crossclamp times, and postoperative renal failure. Root replacement improved MR without mitral repair, but mitral repair further improved the degree of regurgitation.

Joel Dunning talks with Carlos Mestres, Senior Staff Surgeon at the University of Barcelona, member of the International Collaboration on Endocarditis, and Associate Editor of the European Journal of Cardiothoracic Surgery. In this interview, Mr. Dunning and Dr. Mestres take an in-depth look at the treatment of endocarditis.

At the 2013 STS Annual Meeting CTSNet hosted a video roundtable with important perspectives on how valve clinic programs can assist in combating undertreated valvular heart disease. The discussion was moderated by Mike Mack, MD.

Demonstrated in the video is a novel concept for facilitating extensive neochordal repair in mitral regurgitation based on the paradigm “respect rather than resect”. Both leaflet tissue and motion are respected. The leaflet is folded and the neochordae is tied, maintaining free margin and annulus edge-to-edge.

Patient Selection
August 2, 2011
The present a case of a 55 year old male with known mitral valve insufficiency. By transesophageal echo the patient was found to have a ruptured chordae on the P2 segment of the posterior leaflet of the mitral valve. He underwent minimally invasive mitral valve repair via right thoracotomy.

UPDATE: A follow-up video has been added that demonstrates how to manufacture the chordae system. Through a port-access incision, this video demonstrates a technique for placing an echocardiographically pre-measured chordae system to properly suspend the whole of the prolapsed anterior leaflet to its normal level of coaptation.

Artificial chordal replacement allows repair of most bileaflet prolapse and Barlow's valves. This video, presented at the 2004 STS meeting, is still useful today as chords are being used more frequently.
