Ascending Aorta

April 28, 2019
The authors analyzed the STS database for elective aortic root replacement (with several important exclusions) to derive risk factors for mortality and morbidity. A total of 8,807 patients met inclusion criteria.

In 148 patients with type A aortic dissection who underwent emergency total arch replacement, 155 had a conventional elephant trunk procedure (CET, age 65 ± 12 years) and 33 had a frozen elephant trunk procedure (FET, 67 ± 11 years). Early mortality was similar in the two groups (8.7% with CET versus 6.1% with FET).

In this study, researchers from Spain have found that the longitudinal strain of the proximal aorta, as detected by cardiac magnetic resonance, is a risk factor for aortic root dilation rate and aortic events (hazard ratio 1.290, 95% confidence interval 1.123 - 1.481, P<0.001) in patients with Marfan syndrome.

The UK Aortic Group report their experience with the frozen elephant trunk (FET) repair for 66 acute type A aortic dissection (ATAAD) patients using the Thoraflex Hybrid graft at eight UK high-volume aortic centers. The in-hospital mortality was 12% (8/66).
April 14, 2019
The International Aortic Arch Surgery Study Group assessed the safety of total arch replacement (TAR) with frozen elephant trunk (FET) for acute type A aortic dissection (ATAAD) with respect to the risks of operative mortality, stroke, and paraplegia.

Currently no frozen elephant trunk (FET) prosthesis are readily available in the United States. This University of Colorado team developed a surgical technique to simplify the FET procedure, termed the Buffalo Trunk, which obviates the need for a hybrid graft and decreases operating times.

Although computed tomography is the most used imaging technique in the diagnosis of acute aortic syndrome, echocardiography offers complementary information for diagnosis and management in the emergency room, operting rooom, and catheter lab.

Professor Sharma and his team from London investigated the prevalence of an enlarged aorta in 3781 British athletes and 806 control individuals. The mean age was 19 ± 5.9 years, and 63.3% were male. The average time of training was 16.7 hours per week. The follow-up lasted for 5 ± 1.5 years.

To date, there has been no unianimous method to calculate the diameter of the aorta prior to the occurence of type A aortic dissection. In this study, reseachers from Osaka, Japan, reported the efforts in developing equations for estimating the predissection diameters of the ascending aorta and the arch based on postdissection measurements.

Researchers of the Canadian Thoracic Aortic Collaborative (CTAC) analyzed the operative outcomes in 1653 patients (30% women) undergoing thoracic aortic surgery with hypothermic circulatory arrest between 2002 and 2017 in 10 institutions.
