The formation of endotracheal casts comprised of inflammatory debris (plastic bronchitis) is a rare but life-threatening complication of the Fontan procedure.  This summary of 14 pts so affected demonstrated a mean interval to development of plastic bronchitis of 1.5 years.  Casts were treated with outpt t-PA (13 pts) and pulmonary vasodilators.  Les

Fontan connections made using a lateral tunnel (LT) or extracardiac (EC) pathway were evaluated over time using MR. LT pathways generally increased in diameter while EC pathways did not. Normalized LT and EC diameters decreased relative to somatic growth, while descending aortic diameter increased in parallel with somatic growth.

The RV-PA conduit or Sano modification, has recently been reintroduced into the Norwood armamentarium. Many centers, however, have been experiencing conduit obstruction with the Sano. This video demonstrates a modification of the technique used for the RV-PA conduit that has reduced the incidence of conduit obstruction at our institution.
