Tricuspid valve disease

Impressive  decrease of death (by approximaltely 40% !) in 2 years from a small RCT in elective structural heart surgery for degeneration. The assumption is  a benefit  by improvement in right ventricular dynamics

Severe leaflet tethering which is mainly the result of long standing Tricuspid valve regurgitation and RV dilatation is the main predictor of residual and/or recurrent valve insufficiency. We present the Novel technique of Papillary Muscle Cross Control which effectively overcomes the leaflet tethering in a very practical, simple , and reproducible manner by controlled approximation of the anterior papillary muscle to the posterior/septal part of the semi-rigid annuloplasty ring. The papillary cross-control technique is applicable both as a supplementary technique after ring annuloplasty for functional TR with tethered cusps and also for completing the repair in more complex cases of organic valve disease such as Ebstein Anomaly. Our early and late results with this technique has been quite encouraging.

A scholarly narrative on the utility of quantifying the performance of the right ventricle: the RV has been hitherto empirically assessed in many clinical setups.

On May 25, 2021, Dr. David Heimansohn successfully performed the first surgical implantation of biopolymer Tria™ heart valve in a patient with mitral valve disease at Ascension St. Vincent Hospital, Indianapolis, IN.

Holmgren et al. report the long-term outcome of aortic valve replacement patients for bicuspid or tricuspid aortic valve pathologies within a Swedish cohort. In their analysis, patients with bicuspid valve morphology showed an independent association with improved outcome, even matching the general population´s prognosis.

In this video, the authors demonstrate the step-by-step approach to perform septal leaflet augmentation and several neochordae that allowed effective correction of severe tricuspid regurgitation with annular dilation and complex tethering of valve leaflets.

Rheumatic heart disease (RHD) continues to be responsible for major cardiovascular morbidity and mortality in the developing world.  It constitutes a double tragedy because it disproportionately affects economically disadvantaged people and young people, thereby robbing them of the most productive years of life.

IN the case of tricuspid regurgitation caused by mechanical interference of the CIED lead, a bespoke repair aiming to avoid recurrence of mechanical interference is mandatory.

This video demonstrates a fifth sternotomy for triple valve replacement via combined extended Manouguian and Guiraudon vertical transseptal approach.

The authors make a brief overview of tricuspid regurgitation associated with congenital heart defects, including morphology, incidence and echocardiographic evaluation.  The status quo of medical and surgical managements and outcomes are also comprehensively reviewed.
