Dr. Thomas Walther discusses the transapical TAVR Access technique at Dallas-Leipzig Valve 2012.

This article on Type A Aortic Dissection was selected as a top article from 2013 by the 2014 Circulation editors.  The original article is here: http://circ.ahajournals.org/content/128/15/1602.full?sid=a7153f9d-9551-4...

This article describes a standardized and reproducible trans-aortic TAVR approach with the Edwards Sapien valve and Ascendra 3 delivery system.

Authors compared transcatheter aortic–valve replacement (TAVR), using a self–expanding transcatheter aortic–valve bioprosthesis, with surgical aortic–valve replacement in patients with severe aortic stenosis and an increased risk of death during surgery.

In this study the authors looked at the impact of coronary artery disease (CAD) in a single series of 445 patients undergoing TAVI for aortic valve stenosis.  The pre-established primary endpoint of the study was a composite of cardiovascular death, stroke, or myocardial infarction (MI)—at 1 year.

In this manuscript, the authors compare outcomes among patients from the PARTNER trial with diabetes who underwent surgical or transcatheter treatment. Among these patients with diabetes, 6-month and 1-year all cause mortality was lower in the transcatheter group compared with the surgical group. At 2 years this survival benefit was lost.

In this manuscript the authors analyse data from the cohort B (inoperable patients) of the PARTNER trial in order to compare outcomes in two groups of patients, those inoperable for technical reasons and those inoperable for clinical reasons. Patients deemed inoperable for technical reasons had a lower risk profile.

There have been some concerns about outcomes of TAVI in patients with bicuspid aortic valves (BAV). In this retrospective study, outcomes after TAVI in 21 patients with BAVs were compared with those in 447 patients with tricuspid aortic valves (TAV). Device success was lower and 30-day mortality was higher in BAV patients.

In this study, the authors evaluate the relevance of post-procedural-aortic-regurgitation (AR) following transcatheter aortic valve replacement (TAVR) in patients included in the FRANCE 2 registry. They compare outcomes and risk factors for post-procedural AR in balloon expandable (BE) and self-expandable (SE) prosthesis.

Transaortic valve replacement is an alternative to the transfemoral and transapical approaches in an 83-year old male with ischemic heart disease, severe peripheral artery disease and severe symptomatic aortic stenosis.
